The Louisiana Request to Cancel Inscription(s) (This form MUST accompany all cancellation documents.) is a form used to cancel an inscription or registrations for official documents in the state of Louisiana. This form must be submitted along with the other cancellation documents and must be signed by the applicant. The form contains information such as the name and address of the applicant, the type of document being cancelled, and the date of the cancellation. There are two types of Louisiana Request to Cancel Inscription(s): one for cancelling an inscription or registration of a Certificate of Marriage, and one for cancelling an inscription or registration of a Certificate of Birth.
