Louisiana Request to Cancel Mortgage (Authorization or Note Attached) is a legal form that allows a borrower to request the cancellation of their mortgage loan with the lender. This form is used in the state of Louisiana and requires the borrower to provide the lender with a signed authorization or note attached to the request. The lender may then cancel the mortgage if they are satisfied with the borrower’s request and the attached authorization or note.
