The Kentucky Declaration About an Individual Debtor's Schedules is a form that is used in the state of Kentucky to declare that all the debtor's assets and liabilities have been properly listed and that all the required schedules have been filed with the court. This form is usually filed with the court along with the debtor's schedules of assets and liabilities, as well as the Statement of Financial Affairs. There are two different types of this form: the First Kentucky Declaration About an Individual Debtor's Schedules and the Final Kentucky Declaration About an Individual Debtor's Schedules. The First Declaration is filed with the original schedules while the Final Declaration is filed with the amended or modified schedules.
