The petition must include the petitioner’s name, address, date of birth, social security number, the name of the court in which the petition is being filed, a statement by the petitioner that he or she has not been convicted of a felony or misdemeanor involving the use or threat of violence within the last 10 years, and the petitioner’s signature. The petitioner must also provide evidence that he or she is now eligible to possess a firearm. The petition must be served upon the Commonwealth of Kentucky, the Kentucky State Police, and the county attorney of the county in which the petitioner resides.
