The Kentucky Petition-Application for Emergency Appointment of Fiduciary for Disabled Persons is a legal form used to request the appointment of a fiduciary for a disabled individual. This form is used to allow a disabled person to be given authority to manage financial and/or legal matters. The form includes information about the disabled individual, including their name, address, and Social Security number, as well as information about the proposed fiduciary, including their name, address, and relationship to the disabled individual.
