The Kentucky Request for Interpreter-Assistive Technology "Deaf/Hard of Hearing" is a document designed to help individuals with hearing loss receive the appropriate communication services necessary for their needs. It includes sections for identifying the individual's degree of hearing loss, the type of communication services requested, the duration of the service, and the cost of the service. The document is available in both English and American Sign Language (ASL). There are two types of Kentucky Request for Interpreter-Assistive Technology "Deaf/Hard of Hearing": the Standard Request and the Special Needs Request. The Standard Request is for individuals who need an interpreter to assist them in communicating with others. The Special Needs Request is for individuals who require additional accommodations, such as assistive technology, to assist them in their communication needs.
