This form is typically filed with the court when an attorney is taking over representation of a client in a case from another attorney, or when an attorney is continuing to represent a client in a case that has been continued. The form includes the name of the attorney, the name of the client, the case number, the court in which the case is being heard, and the attorney’s signature.
