Indiana Plan Modification, Motion, and Notice (Combined) is a set of forms used to modify an existing child support order in Indiana. It is a combination of the Motion to Modify Child Support and the Notice of Filing of Motion to Modify Child Support forms, which are both required when modifying a child support order in Indiana. The forms are used to notify the other party of the motion and to provide the court with the necessary information to make a decision. The forms must be filed in the county where the existing child support order is in effect. The forms ask for information such as the existing child support order, the proposed changes to the order, and any other relevant information. There are two types of Indiana Plan Modification, Motion, and Notice (Combined): 1) Form 1-P-S and 2) Form 2-P-S. Form 1-P-S is the form used when the modification is to reduce the amount of child support owed, and Form 2-P-S is the form used when the modification is to increase the amount of child support owed.
