The Illinois Income Withholding For Support-Form (IW-2) is a document used to establish withholding of income for child support payments. The form is used by employers to deduct child support payments from an employee’s wages. It is commonly used when a non-custodial parent is required to pay child support, and when an employer is ordered to withhold income from an employee’s wages for payment of child support. The form is completed by the employer, and includes information such as the employee’s name, address, Social Security number, and the amount to be withheld. The form is then submitted to the Illinois Department of Healthcare and Family Services (HFS). There are two types of IW-2 forms: the IW-2 for Support Order Withholding and the IW-2 for Income Withholding Order for Support.
