The Illinois Civil Cover Sheet (IND) is a form used in the state of Illinois to provide the court with information about civil cases. It is a mandatory form that must be filed with the clerk’s office at the time a plaintiff or defendant files a complaint, counterclaim, cross-claim, third-party complaint, or other civil pleadings. The IND contains information about the parties involved in the case, the nature of the claim, the amount of damages sought, and the relief requested. The information provided on the form is used by the court to assign the case to the appropriate division and judge. There are two types of IND forms: 1) the original IND, which is used for first filings; and 2) the Amended IND, which is used for subsequent filings. The Amended IND must be filed in order to make changes to the original IND, such as updating information, adding parties, or changing the relief requested.
