The Illinois Affidavit for Change of Name-Address (Unclaimed Check) is a document used to change the name and/or address of the recipient of an unclaimed check issued by the State of Illinois. This affidavit must be completed by the original recipient of the check and must be sworn to and notarized. It is used to update the name and/or address of the payee on the check and to transfer the unclaimed check to a new payee. There are two types of Illinois Affidavit for Change of Name-Address (Unclaimed Check): the affidavit for an unclaimed check issued to an individual person, and the affidavit for an unclaimed check issued to a business entity. Both forms require the original payee to provide their name, address, and Social Security number, as well as the name and Social Security number of the new payee. The forms also require the original payee to provide the check number, the date the check was issued, and the amount of the check. Finally, the forms require the original payee to provide a notarized signature.
