Iowa Confidential Information — Social Security Number Form For Sec. 598.22b And 602.6111(2) is a form used by the State of Iowa to collect and store Social Security Numbers (SSNs) and other confidential information. This form is used to protect the confidentiality of SSNs and other confidential information collected by the State of Iowa. The form also serves to protect individuals from potential identity theft. There are three types of Iowa Confidential Information — Social Security Number Form For Sec. 598.22b And 602.6111(2): an Employer Form, an Individual Form, and an Agency Form. The Employer Form is used by employers to provide confidential information to the State of Iowa; the Individual Form is used by individuals to provide confidential information to the State of Iowa; and the Agency Form is used by agencies to provide confidential information to the State of Iowa.
