The first is the Income Affidavit, which includes information about gross and net income, income deductions, and other income sources. The second is the Child Support Worksheet, which provides a calculation of the amount of child support to be paid. The third type is the Guidelines Worksheet, which provides a calculation of the parties' respective contributions for child care expenses.
