The Florida Affidavit To Credit For Direct Support Payments is a document used to help the recipient of a child support payment receive a credit for the support payments made. This document is required when a non-custodial parent, who is obligated to make payments, fails to do so. It is used to document the missed payments and ensure the custodial parent is able to receive credit for the payments that have been made. There are two types of Florida Affidavit To Credit For Direct Support Payments: a Form 12.902(e) and a Form 12.905. The Form 12.902(e) is used when the non-custodial parent makes payments directly to the custodial parent. The Form 12.905 is used when the non-custodial parent makes payments through the Florida Department of Revenue's Child Support Program. Both forms require information such as the names and addresses of both parents, the amount of the payment, the payment date, and a description of the payment.
