District of Columbia Amendment or Cancellation of Statement of Authority for Domestic Limited Liability Company is a form that is used to alter, amend, or otherwise modify the existing statement of authority for a domestic Limited Liability Company (LLC) registered in the District of Columbia. This form allows a domestic LLC to add, delete, or change the name, address, or manager information of the LLC. There are two types of District of Columbia Amendment or Cancellation of Statement of Authority for Domestic Limited Liability Company: Amendment of Statement of Authority and Cancellation of Statement of Authority. The Amendment of Statement of Authority form is used to modify the existing statement of authority, while the Cancellation of Statement of Authority is used to terminate the existing statement of authority. Both forms must be completed in full and filed with the appropriate office in the District of Columbia.
