The Colorado Application For Expedited Hearing — One Time Change Of Authorized Treating Physician is an application form used to request an expedited hearing in order to change an authorized treating physician. This form is used by patients who wish to change their current treating physician to a different authorized treating physician. The form includes the patient’s name, address, phone number, and other contact information, as well as information about the current treating physician, the requested new treating physician, and the reason for the change. There are three types of Colorado Application For Expedited Hearing — One Time Change Of Authorized Treating Physician: 1) Initial Request, 2) Request for Post-Hearing Review, and 3) Request for Rehearing. The Initial Request is used when the patient is requesting to change their current treating physician to a different authorized treating physician. The Request for Post-Hearing Review is used when the patient is requesting a review of the decision made at the original hearing. The Request for Rehearing is used when the patient is requesting a rehearing of the decision made at the original hearing.
