The Arizona Affidavit of Notice of Hearing Regarding Discharge Termination and/or Release of Funds in an is a document that must be filed with a court when a party is requesting funds to be released and/or discharged after a hearing. This document provides notice to the parties involved that a hearing is scheduled in the matter. The affidavit must be signed by the party requesting the hearing, and must be notarized. The three types of Arizona Affidavit of Notice of Hearing Regarding Discharge Termination and/or Release of Funds in and are: (1) Discharge Termination Affidavit; (2) Release of Funds Affidavit; and (3) Combined Discharge Termination and Release of Funds Affidavit.
