Alaska Motion & Affidavit to Convert Dissolution to Divorce, SHC-1330, is a document used for couples who initially agreed to a dissolution of marriage but wish to convert that dissolution to a divorce. The motion and affidavit document is filed with the court to request that the dissolution of marriage be converted to a divorce. The motion and affidavit includes the names of the parties, the case number, the type of case, the date of the original dissolution and the reasons for converting the dissolution to a divorce.
