The Alaska Complaint by Mother to Disestablish & Establish Paternity, SHC-125, is a legal document that is used to terminate the presumed father's legal rights and responsibilities to a child and to establish a new legal father for the child. It is typically signed by the child's mother and filed in court, along with a supporting Affidavit of Paternity, in order to seek a court order that sets forth the rights and responsibilities of the new legal father. There are two types of Alaska Complaint by Mother to Disestablish & Establish Paternity, SHC-125: Voluntary Acknowledgement of Paternity and Judicial Established Paternity. The Voluntary Acknowledgement of Paternity is usually signed by the mother and the presumed father, and filed with the State of Alaska. The Judicial Established Paternity is usually requested by the mother, and requires a court hearing where the mother and the new legal father must appear and testify.
