Alaska Civil Rule 26.1 Questionnaire, SHC-1010 is a form used in civil litigation in Alaska to simplify and expedite the discovery process. It is designed to identify potential areas of dispute and narrow the scope of discovery. The form consists of three parts: Part I contains basic information about the parties and the case; Part II contains interrogatories and requests for production of documents; and Part III contains requests for admissions. There are two types of Alaska Civil Rule 26.1 Questionnaire, SHC-1010: the initial questionnaire and the supplemental questionnaire. The initial questionnaire is completed by the plaintiff and sent to the defendant. The supplemental questionnaire is completed by the defendant and sent to the plaintiff. Both questionnaires must be answered truthfully and completely.
