The South Dakota Notice of Withdrawal From Registration as an Investment Adviser (ADV) is a form that must be completed and submitted to the South Dakota Division of Securities in order to withdraw a company's registration as an investment adviser. There are two types of ADV forms that can be used, depending on the circumstances of the adviser's withdrawal. The first type is the ADVW-1 form which is used when an investment adviser is withdrawing from registration voluntarily. The second type is the ADVW-2 form which is used when an investment adviser is withdrawing from registration for disciplinary reasons. Both forms must be completed with information about the adviser, the withdrawal, and the reason for the withdrawal. The forms must be signed and notarized before being submitted to the South Dakota Division of Securities.
