Oklahoma Release and Authorization (ORA) is a legal document that allows a legal representative or the principal (the individual who is authorizing the release of information) to authorize and release specified personal information to a third party. This type of document is most commonly used in situations where the principal may not be able to provide information directly to the third party due to physical or legal restrictions. ORA includes two types of documents: the Oklahoma Release and Authorization Form and the Oklahoma Release and Authorization Verification Form. The Oklahoma Release and Authorization Form must be completed and signed by the principal in order to authorize the release of information. The Oklahoma Release and Authorization Verification Form must be completed and signed by the third party to whom the information is being released. The information released may include but is not limited to: name, address, date of birth, Social Security number, driver’s license number, bank account information, and medical records.
