Connecticut Resignation of Guardian or Temporary Custodian (New 10/15 and Rev. 10/17 re PC-583CI)) is a legal form, also known as a Connecticut Resignation of Guardianship or Temporary Custody, that is used in the state of Connecticut. This form is used to inform the Probate Court of a guardian's or temporary custodian's intent to resign from their appointed position. This form must be signed and notarized by the resigning guardian or custodian. There are two versions of this form: the New 10/15 version and the Rev. 10/17 version. Both versions require the same information, which includes the name of the guardianship or temporary custody, the name of the person resigning, and the date of resignation. The New 10/15 version requires the signature of the guardian or custodian, whereas the Rev. 10/17 version requires the signature of the guardian or custodian and the signature of the Probate Court. By signing and notarizing this form, the guardian or custodian is legally resigning from their appointed position.
