Designate one (and only one) account to receive any excess funds left over after all direct deposits are processed. All Employees must complete and have on file AUTHORIZATION AGREEMENT FOR AUTOMATIC DEPOSITS (ACH Credits) form. Your bank's signed, official account verification document indicating your name, address, bank name and routing number, and account number. Do not send this form to Intuit. Employees must attach a voided check for each of their accounts to help verify their account numbers and bank routing numbers.
