When applying for a certificate you must present an unexpired government-issued photo identification at time of application. If you are requesting that the certificate be mailed to you, you must provide a form of government-issued photo identification that also provides proof of address.

Proof of U.S. Citizenship - Certified Birth Certificate (must have the full names of both parents) from the state where you were born, (Birth Notification, Birth Registration or Hospital Certificates are not acceptable) or a previous U.S. Passport or your Naturalization Certificate.
