This application should be completed by the student, the student's legal guardian and the student's prospective employer. The completed application should be submitted to the school in which the student is enrolled.
NOTE: When you submit your application for a work permit to your school district, both the 14 or 15-year-old AND a parent/ guardian must be present. The student must also bring proof of age, either a birth certificate or State of Ohio ID.
Minors ages 14-15 may not work more than 3 hours per school day or more than 18 hours per week. Those hours are limited to between a.m. and p.m. Minors ages 14-15 may not work more than 8 hours during a day when school is not in session or 40 hours per week.
Requirements & Expectations Students with irregular attendance, or less than 93% attendance in school, may be denied a work permit or the work permit may be revoked. 93% positive attendance also includes tardies, individual classes missed and total school hours missed.
A: If a minor is homeschooled and needs to obtain a Work Permit, they can get the school portion of the application filled out by the school district (their main office) within which the minor resides.
The Application for Minor Work Permit is to be filled out by the student and signed by a parent and guardian.
