If you do not have health insurance, Covered California can help you determine if you qualify for Medi-Cal or federal subsidies, and can provide you with coverage options and plan costs. Simply submit your information, and we'll get back to you about if you qualify for Medi-Cal through Covered California. This is your fastest option if you're interested in signing up for Medi-Cal.
Submitting Claims The claim submission timeframe for Contra Costa Health Plan is 180 days from the date of service, or primary explanation of benefits (EOB), for both contracted and non-contracted providers. Claims received after 180 days will be denied for untimely filing.
As a California resident, you should carry insurance throughout the year with no gaps in coverage of 90 days or more. Otherwise, you may face a tax penalty when you file your tax return. There are some exemptions to California's penalty, which we'll discuss later.
What You Need to Do. Call Health Care Options at 1-800-430-4263 to request your Medi-Cal Choice Enrollment Form. Look for the Medi-Cal Choice enrollment form in the booklet. You can also send in the postcard that you received from Health Care Options to get your Medi-Cal Choice form.
If you do not show proof of insurance when asked, you will get a ticket. If you do not have any insurance, your license may be suspended and your vehicle could be impounded.
