It is legit, it's for the Immigration Dept. They don't ask for a social number or drivers lisc. But will ask for Passport number. They ask for your email and home address and where you're staying when on island. Dont worry, you won't be signed up for an email list and forever get spam in filling the form out.
For Visitors U.S. citizens are required to present a valid U.S. passport for entry into Bermuda and re-entry into the U.S. In other words, U.S. citizens must have a U.S. passport valid for the duration of their travel to Bermuda.
