The document is an Affidavit of Residence required for parents or court-appointed custodians to verify the residency of a child attending Wake County Public Schools. It includes sections for personal information, signatures from both the parent and residence provider, and requires notarization.
1st – 12th grade: Certified birth certificate. Record of immunizations (originals from the doctor or health department) - must be within one year of enrollment. Health Assessment completed by a doctor (only applies to students who have NOT attended a North Carolina Public School in the past)
Parents and/or guardians MUST come to the school in order to withdraw the student from school. Please bring a valid photo ID, to verify that you are the legal guardian of that child(ren).
Contact Central Records at 919-694-0585 if you are a former student. Contact your high school if you are a current student requesting a transcript. Contact the WCPSS school of the K-12 student transferring to your school or district.
When you move, you must contact the school district in your new location to learn how to transfer your child. Most communities require that you fill out a transfer form or transfer request. You must also provide documents like school records, immunization records, and proof of residence to complete the transfer.
Register for classes Email ccp@waketech or set up a virtual advising appointment to get the form. Remember, CCP students are allowed to take only courses within their approved pathway(s), as well as within allowable credits and program-specific restrictions.
This is an important choice, widening parents' options and ensuring that their zip code isn't the sole determiner of their education. Unfortunately, North Carolina families generally do not have open enrollment options. There are, however, a few circumstances where a transfer might be possible.
