Contact the school's office and explain you would like to have your child evaluated for an IEP. That gets the process started. A Special Education teacher or school counselor or school psychologist will contact you about forms to fill out and a questionnaire to answer.
In NYC, the special education preschool and school age children are coordinated by the Department of Education (DOE).
