When filling out the FMLA forms, be sure to provide accurate and complete information about your need for leave. Include information about your health condition or the health condition of your family member, the expected duration of your absence from work, and any other relevant details.
Documentation of Family Relationship Employers may not request a certification for FMLA leave to bond with a newborn child or a child placed for adoption or foster care. However, employers may require employees to provide reasonable documentation of a family relationship.
Visit the FMLA website to find and print out the FMLA form. Have your employer complete section 1, then fill out the required information in section 2, like your full name. Meet with your healthcare provider and have them fill out section 3, then return the completed form to your employer.
Once a student has enrolled for at least one semester, the student has the option of not enrolling for one semester. If a student opts not to enroll for one semester, no paperwork is required. Please note that newly enrolled students must attend for one semester before they are eligible for this semester leave.
Withdrawal Undergraduate students will need to submit the Withdrawal from Semester Request which is available on Undergraduate Education's Petitions page. Graduate students need to file the Graduate Petition for Course Drop/Semester Withdrawal. Go to the GAPE Forms page and look under Special Petitions.
