When filling out the FMLA forms, be sure to provide accurate and complete information about your need for leave. Include information about your health condition or the health condition of your family member, the expected duration of your absence from work, and any other relevant details.
Documentation of Family Relationship Employers may not request a certification for FMLA leave to bond with a newborn child or a child placed for adoption or foster care. However, employers may require employees to provide reasonable documentation of a family relationship.
Visit the FMLA website to find and print out the FMLA form. Have your employer complete section 1, then fill out the required information in section 2, like your full name. Meet with your healthcare provider and have them fill out section 3, then return the completed form to your employer.
