Explain that the job is making you especially depressed and anxious and that you can't face going back. Tell them you have a notice period and ask them to sign you off for the duration of it; if they agree with your assessment that the job is putting you in ill health then they'll probably agree to do so.
Write a simple two sentence resignation letter addressed to your boss. In the first sentence, state that you are resigning your position. In the second sentence, state that your last day will be two weeks from the date of your letter. Print your letter on plain paper, sign it and copy it for your files.
While you can give your employer a clear reason as to why you are leaving, you don't need to explain everything. For example, if your reason for leaving is a health issue, you can write “due to illness” in your letter.
