Name and social security number changes require a photocopy of the student's social security card. Contact the Student Enrollment and Services Office for further information. The Master Record Change form is available in the Admission Forms section of this website.
To have your name officially changed, you will need to complete and submit a name change form (PDF) to the Registrar's Office with appropriate documentation.
If you want to change your name on your college diploma, you must notify the registrar's office. The registrar will send you a new diploma within six weeks. You may need to fax your documents or visit in person to change your name. The registrar will verify that your name is correct before issuing the new diploma.
All current classes in progress, completed or needed to finish the degree plan are within the Cougar Compass audit. To change your major, log into your CougarWeb account and under the Home Tab select REQUEST CHANGE OF MAJOR.
Students are able to withdraw from a class (or classes) on CougarWeb. CougarWeb will process total withdrawals.
