The Rhode Island Cover Sheet is a form used to accompany documents filed with the Rhode Island Secretary of State. The form must be completed and submitted with any documents filed with the Secretary of State's office, including Articles of Organization, Amendments, Annual Reports, Mergers, and other documents related to the formation and maintenance of a business entity. There are three types of Rhode Island Cover Sheet: Domestic (for domestic business entities), Foreign (for foreign business entities), and Non-Profit (for non-profit entities). Rhode Island Cover Sheets must be signed by the authorized representative of the business entity, and must include the name of the business entity, the name of the authorized representative, the business entity type, and the document being filed.
