The Arizona Affidavit of Financial Information is used by the court to determine whether a party has the financial means to pay for court-ordered child support, spousal maintenance, or attorney's fees. It is also used to collect information for the Arizona Child Support Guidelines. The affidavit must be signed and notarized and filed with the court before a court order is entered. There are two types of the Arizona Affidavit of Financial Information: a short form and a long form. The short form includes basic financial information such as income, debt, assets and liabilities. The long form is more detailed and includes more in-depth financial information such as business income, rental income, and investments. Both forms must be completed and submitted to the court.
