The Civil Cover Sheet is filed at the same time as the Petition and/or Complaint. It lists the parties involved, as well as the type of cause of action and amount in controversy. When filed with the court, the clerk of court will issue a Cause Number for that case, and that number is to be used when referring to that case, throughout its entirety. How to fill out Civil Case Information Sheet - Texas Rule Of Civ. Proc. 78a?
