The Wisconsin Notice of Potential Eligibility to Receive Vocational Rehabilitation Services is a form used by the Wisconsin Department of Workforce Development's Division of Vocational Rehabilitation (DVR) to determine whether an individual is eligible to receive vocational rehabilitation services. The form consists of two parts. The first part contains questions related to the individual's disability and vocational goals, and the second part contains questions related to the individual's education and employment history. The form must be completed and signed by the individual and returned to the DVR in order for the individual to be considered for vocational rehabilitation services. There are three types of Wisconsin Notice of Potential Eligibility to Receive Vocational Rehabilitation Services: 1. Initial Notice of Potential Eligibility for Vocational Rehabilitation Services: This is the first form that must be completed and submitted to the DVR in order to be considered for vocational rehabilitation services. 2. Renewal Notice of Potential Eligibility for Vocational Rehabilitation Services: This form is used to renew an individual's eligibility for vocational rehabilitation services. 3. Reinstatement Notice of Potential Eligibility for Vocational Rehabilitation Services: This form is used to reinstate an individual's eligibility for vocational rehabilitation services after their eligibility has expired.
