The Wisconsin Affidavit of Service of SC-500 Summons and Complaint Individual/Sole Proprietorship/Partnership — Non-Eviction (Small Claims) is a document used by a party to a small claims action in Wisconsin to certify that service of process was made on the other party. The Affidavit of Service must be completed and filed with the court and signed in front of a notary public. There are two types of Affidavit of Service forms: one for service of process on an individual or sole proprietorship, and one for service of process on a partnership. The Affidavit of Service must include the name of the person served, the date the document was served, the method of service used, and a certification that the document was served in accordance with Wisconsin Statutes. The Affidavit of Service must also include the physical address, mailing address, and telephone number of the person served.
