The Wisconsin Order for Extension of Dispositional Order/Consent Decree (Out-of-Home Placement Only) — Indian Child Welfare Act is a court order issued by a Wisconsin court that extends the period of out-of-home placement of an Indian Child, in accordance with the Indian Child Welfare Act (IOWA). The Order must be obtained by the Indian Children's Placement Agency, on behalf of the Indian child, in order to extend the period of out-of-home placement beyond the initial order. The Order must include the following: name and address of the Indian child, the name and address of the Indian child's parent or guardian, the name and address of the Indian child's tribe, a statement that the child remains an Indian child, the period of extended out-of-home placement, and the requirements for continued out-of-home placement. The Order may also include a requirement for the Indian child's parent or guardian to be informed of any change in the child's placement and to be given an opportunity to be heard.
