Wisconsin Third Party — JuvenileRecords Request is a request by a third party (such as an employer, government agency, or educational institution) to obtain criminal and/or juvenile records from the State of Wisconsin. The types of records that may be requested include: juvenile court records, juvenile disposition reports, juvenile delinquency records, and juvenile commitment records. The request must include the name of the juvenile, the date of birth, and the name of the court or agency that has jurisdiction. The request must also include a release of information signed by the juvenile or the juvenile's legal guardian. The request is then sent to the Wisconsin Department of Justice, which will review the request and provide the records, if available.
