The Wisconsin Exhibit List Continuation Sheet is a form used by attorneys to provide more information about exhibits they intend to use during a trial or hearing. The form is used to supplement the Wisconsin Exhibit List, which is the primary document used to list all exhibits to be introduced into evidence. The Continuation Sheet lists additional exhibits, such as those that have been newly discovered or those that have been added after the Wisconsin Exhibit List has been filed. The form must be signed by the attorney representing the party who created the list and by the attorney representing the opposing party. There are two types of Wisconsin Exhibit List Continuation Sheets: Form ETHIC (Civil) and Form EXACT (Criminal).
