The document must include the name of the preparer, the name of the debtor, the proposed fee arrangement, and the bankruptcy petition preparer’s signature. The document must also include a statement that the preparer is not an attorney or a bankruptcy trustee and that the debtor should seek the advice of an attorney or a bankruptcy trustee. There are two types of Wisconsin Disclosure of Compensation of Bankruptcy Petition Preparer: a one-time fee disclosure and an hourly fee disclosure. The one-time fee disclosure must include the amount of the fee and the services to be provided. The hourly fee disclosure must include the hourly rate, the estimated number of hours to prepare the documents, and the total estimated fee.
