The Wisconsin Corporate Ownership Statement (Adversary Case) is a form filed with the Wisconsin Department of Financial Institutions (DFI) by a corporation when it is named as a defendant in a civil action or adversary proceeding. The statement requires the corporation to provide information about its directors, officers, and shareholders. The statement must be filed within 20 days of the commencement of the action or proceeding and must be updated every 30 days thereafter. The statement must include the names and addresses of the corporation’s directors, officers, and shareholders, as well as the percentage of ownership held by each. There are two types of Wisconsin Corporate Ownership Statements (Adversary Case): the Adversary Case Statement of Ownership and the Adversary Case Statement of Directors and Officers.
