The Washington Acknowledgment of Notice of Rights — Criminal Complaint is a form used by Washington State Courts to confirm that a defendant has received a copy of the criminal complaint and has been informed of their rights. The form includes the defendant's name, the date, the court name, the name of the prosecutor, the signature of the defendant and a witness, and the signature of the judge. There are three types of Washington Acknowledgment of Notice of Rights — Criminal Complaint forms: the original, the amended, and the supplemental. The original form is used when a defendant is charged with a crime. The amended form is used when additional charges are added to the criminal complaint, and the supplemental form is used when the defendant waives their right to counsel.
