The Utah General Business Supplemental Questionnaire is an additional set of questions that must be completed by businesses when submitting a business registration to the Utah Division of Corporations and Commercial Code. The questionnaire is designed to capture additional information about the business, such as the type of entity, registered agent, and ownership information. There are two different versions of the Utah General Business Supplemental Questionnaire: one for Domestic Entities, and one for Foreign Entities. The Domestic Entity form requires information on the type of entity, registered agent, principal office address, and the names and addresses of all owners. The Foreign Entity form requires information on the type of entity, registered agent, address of the entity’s principal office, and information on all owners, including foreign owners, if applicable. Both forms must be completed and submitted along with the business registration in order for the registration to be processed.
