The Utah Order Establishing Facts of Birth is an official court order issued by a Utah court that confirms the birth details of a person. It is commonly used to obtain a new birth certificate or to correct an existing birth certificate. There are three types of Utah Order Establishing Facts of Birth: Adoption Order, Delayed Birth Certificate Order, and Amendment Order. An Adoption Order is issued when the court has determined that the birth certificate should reflect the adoptive parents' details rather than the birth parents. A Delayed Birth Certificate Order is issued when the court has determined that the birth certificate should reflect the actual date and place of birth. An Amendment Order is issued when the court has determined that the birth certificate should be amended to reflect a change in the name or other details of the person. All three of these orders must be signed by a judge and will be issued on official court letterhead.
