The Utah Involuntary Checklist includes criteria such as the individual's current mental state, history of mental health issues, and any dangerous behaviors or threats they may have made. Depending on the severity of the individual's condition, there are three types of Utah Involuntary Checklist: Level 1, Level 2, and Level 3. Level 1 Checklist is used to assess whether the individual is in imminent danger of harming themselves or others, Level 2 Checklist is used to determine if the individual needs' inpatient care, and Level 3 Checklist is used to assess if the individual needs intensive treatment.
