Civil Process Request Form

The Texas Civil Process Request Form is a legal document used to initiate service of process in Texas civil courts. It is used to provide notification to the defendant of a pending legal action, such as a lawsuit, and to request the court to issue a summons to the defendant. There are three types of Texas Civil Process Request Forms: Personal Service Form, Non-Personal Service Form, and Posting/Published Service Form. The Personal Service Form is used when the defendant is located in the same county where the lawsuit is being filed. The Non-Personal Service Form is to be used when the defendant is located outside the county where the suit is being filed. The Posting/Published Service Form is used when the defendant cannot be located or is avoiding service of process. This form requires the plaintiff to post notice of the lawsuit in a public area and publish notice of the suit in a newspaper.
