Texas Form A-211 (Lloyd's) is a legal document issued by the Texas Department of Insurance that is used by insurers to provide proof of workers’ compensation insurance coverage. This form is also referred to as the "Certificate of Insurance," "Workers' Compensation Insurance Certificate," or "Lloyd's Form." It must be completed by the insurer and provided to the employer to demonstrate compliance with the Texas Workers' Compensation Act. The form provides the employer with a summary of the insurance coverage provided, including the name of the insured, policy number, policy limits, and the company providing the coverage. It also provides details on who is covered under the policy, including employees, subcontractors, and temporary workers. There are two types of Texas Form A-211 (Lloyd's): A-211/Lloyd's and A-211/Lloyd's Plus. The former is the basic form, while the latter provides more comprehensive coverage such as occupational accident coverage.
