A Tennessee Affidavit of Indigence is a legal document that is generally used in cases where a person is unable to pay court costs, fines, and/or fees due to financial hardship. The affidavit must be completed and signed in the presence of a notary public and filed with the court in order for the court to consider whether the person is truly indigent and therefore eligible to have the costs waived. There are two types of Tennessee Affidavit of Indigence: Form SF-054, which is used to apply for assistance in paying court costs, fines, and/or fees; and Form SF-056, which is used to apply for a waiver of court costs, fines, and/or fees. Both forms require the person to provide detailed information on their income, assets, and financial obligations in order to be approved.
